REGISTRATION INFORMATION

When:  August 7^th and August 14^th (10:00am-2:00pm)

Where:  Our Lady of Guadalupe School (under school awning) 40382 Fremont Boulevard, Fremont

Softball Registration Fee:  $50.00*

*NOTE:  A discount will be applied if signed-up for two or more sports or signed-up to fill one of the Board Tasks.

Please Bring the Following:

• Completed Registration Packet:

Parental Permission

Health Authorization

Release Form

Code of Conduct

• Proof of Residency (does not apply to OLG or Religious Education Students)

PG&E Bill or Water Bill

• Copy of Birth Certificate (new players to OLG CYO Sports Program)
• Checkbook to pay fees payable to OLG-CYO
