- OLG CYO Important Information Sheet (as needed by by each sport)
- Supplemental Form – CYO Eligibility (as needed by Non-school and Non-Faith Formation Players)
2. Proof of Residence (Does not apply to OLG School and Faith Formation (CCD) students)
- Utility Bill (PG & E, water bill, garbage bill or landline phone bill – no cell phones), AND
- Letter from your child’s school on letterhead stating the address on file for your child (see supplemental form for CYO Eligibility)
3. Proof of Birth: Birth Certificate, Baptism Certificate or Passport Copy (New Players and New Cheerleaders only, excluding OLG School and OLG Faith Formation students)
4. Checkbook or cash to pay fees – All program fees are payable at the time of registration. (Checks payable to: OLG CYO.)
NOTE: REGISTRATION IS NOT COMPLETE UNTIL ALL FORMS ARE RECEIVED and ALL FEES ARE PAID.
